Daily Self-Check Survey
In partnership with the UT Health Science Center, the UT System is now offering a self-check survey for employees to use before they report to work. It was developed with criteria from the U.S. Centers for Disease Control and Prevention (CDC).
The self-check survey is only intended for University of Tennessee employees and requires a NetID and password to access. No data is recorded or collected from this tool. A timestamp is registered at login and upon completion of the survey, to monitor how often the app is used.
In addition to this self-check survey, please take your temperature every day before reporting to work. If your temperature is greater than 100 degrees F, please stay home and call your supervisor.
