General Liability Incidents
All incidents involving personal property damage or injury, including incidents due to any alleged university negligence, should be reported within 24 hours of the incident date. These reports allow Risk Management to identify health, safety or security issues for investigation(s) and provide claim reporting instructions.
Some examples include:
- Damage to items owned by a student
- Volunteer or visitor falls on a sidewalk, stairs or parking lot
- Near misses that occur on UT property

On-the-Job Injuries
The University of Tennessee’s Workers’ Compensation program provides medical and wage benefits to employees who experience job-related injuries. If you sustain an on-the-job injury, you must report it to CorVel within 24 hours using the steps outlined below. For questions or assistance with the reporting process, the UT System Office of Risk Management is here to help.

The Reporting Process
Injured Worker – You start the reporting process:
- Report your injury to your supervisor when it happens.
- Report your injury to CorVel (even minor injuries)
- Call 1-866-245-8588 Option #1 (nurse line)
- If you need medical care, the nurse will send you to an authorized doctor. You MAY NOT treat with an unauthorized provider!
- DO NOT go to the doctor before you report to CorVel.
- Complete an Incident Report online via the Origami Portal
- You will receive an email confirmation from Notifications@OrigamiRisk.com
Supervisor – You finish the reporting process:
- You will receive email notification from Notifications@OrigamiRisk.com of the new injury after the injured worker’s submission is complete.
- Follow the instructions in the email to submit your Supervisor Statement and complete the reporting process.
- Follow up with your injured worker for the doctor’s return to work status. Contact your campus Workers’ Comp Coordinator to process the return to work.
For Life-Threatening On-the-Job Injuries
Call 911 or go to the nearest ER immediately!
Supervisor – You must report emergency on-the-job injuries on behalf of your injured worker:
- Firstly, ensure your injured worker has appropriate medical care (911, nearest ER)
- Call immediately to report your worker’s injury to CorVel (24/7):
- 1-866-245-8588, Option #2
- Report the incident to:
- Your Campus or Department Safety Officer
- Your Supervisor
- UT System Office of Risk Management
Injured Worker – You must initiate the online reporting process as soon as possible:
- Obtain the CorVel claim number from your supervisor
- Complete an Incident Report online via the Origami Portal

Auto Accidents
The State of Tennessee self-insures The University of Tennessee for automobile liability.
This means people who use university vehicles in the scope of their employment or authorization to conduct university business are protected up to $1 million per occurrence for negligence resulting in bodily injury and property damage to third parties.
The state provides liability coverage for injury to third parties as long as the employee’s conduct was not (1) outside the scope of the employee’s employment; (2) willful, malicious, criminal, or involved personal gain; or (3) grossly negligent.

At the Scene of an Accident
- Never admit fault to anyone and/or offer to pay for any loss incurred by another party resulting from the accident.
- Call 911 for medical attention and a police report. Report any injuries if necessary.
- Exchange contact and insurance information with the other driver(s) involved
- Obtain contact (name, address, phone number) information of every witness.
- If the vehicle is towed from the accident scene, determine where the vehicle will be taken and what may be necessary to retrieve the vehicle.
- Take clear photos of all vehicles involved.
After an Accident (Reporting)
- Call the State of TN Auto Accident Call Center (855-253-0629) immediately to report the incident, then call your supervisor and follow your department’s applicable procedures.
- Be prepared to provide the following information to the Call Center:
- Incident date, time, and location.
- University driver’s name, phone number, and email.
- Any/all other drivers’ names and phone numbers, plus emails if available.
- Any other involved passengers’ names and phone numbers, plus emails if available.
- Vehicle information for all vehicles, including license plate number and/or VIN.
- Your supervisor’s name, phone number, or other contact information.
- Confirm if any fatalities or non-fatal injuries occurred.
- Be prepared to provide the following information to the Call Center:
- Report to Office of Risk Management by using the Origami reporting link above.
For those driving a personal vehicle on University business, you may have to complete the Owner/Driver Vehicle Accident Report.
If You Hit an Unoccupied Vehicle
- Fill out a Damage Notification Card and leave it on the unoccupied vehicle.
- Call the State of TN Auto Accident Call Center immediately at 855-253-0629 with the date, time, and location of the incident, as well as any additional information you may have.
- Take photos of both vehicles, as well as the license plate and VIN tag of the other vehicle (on the driver-side dash, near the windshield).
